It is very common for people with chronic illnesses to be on complicated therapeutic regimens, consisting of multiple prescription medications. AllCare has ample experience dealing with patients who are taking as many as 25 to 30 prescriptions daily. It is important for the pharmacy to manage patient profiles by ensuring that prescriptions remain valid, have refills, and are not expired. The pharmacy must also manage drug interactions or other disease-compromising attributes. It is equally important for patients to make sure that they take their medications properly and in a timely manner.

AllCare’s objective is to make sure that all patients’ prescription profiles are appropriately maintained and monitored.

We make sure that all prescriptions are in good standing. A prescription is in good standing when it has a valid date and prescriber AND is appropriate in:

• Dose: what strength?
• Quantity: how much is given?
• Route of administration: how it is taken or applied?
• Frequency: how often it is taken?
• Duration: how long it is to be taken?
• Therapeutic intent: what it is to be taken for?

Not only must prescriptions be in good standing, they must also be without significant contraindication.

Contraindications can include:

• Drug allergies: allergic reaction to drug or related substance.
• Disease state interactions: not well tolerated with certain medical conditions.
• Duplication of therapy: drug can cause additional side effects without clinical benefit.
• Drug-to-drug interaction: reaction between two drugs that is unintended or undesired.

Even when all these criteria are met patients can still experience profound side effects from their medication regimens. We emphasize the importance of patient counseling as an integral part of medication management. Patients need to know what they can expect from their medications, how they can mitigate side effects and which reactions require immediate medical attention. Answering and verifying the patient’s understanding of these simple questions can have a lasting impact on compliance and adherence.

Patients need to make sure that they take their medications appropriately. Depending on the medications this can differ greatly. Some need to be taken with food, on an empty stomach or with a full glass of water. While many medications may need to be taken together, others must be separated by a specified period of time to provide the greatest benefits. For patients on multiple medication regimens, it can be quite overwhelming. Lack of understanding and support often leads to noncompliance. That is what AllCare’s adherence regimen provides—the needed support to understand and deal with complex medication regimens.

We want you to receive your medication in a timely manner. This is why we have implemented a comprehensive adherence monitoring program for each of our patients. This program can include:

Courtesy phone calls:

• We call our patients a few days before their medication refills are due.
• We call our patients if their medications are not picked up on time and offer our Free Delivery* option.

Automatic refill option:

• Your prescriptions are filled every month on a fixed schedule.

Communication with providers:

• We contact providers to renew prescriptions before they expire or on the last refill.
• We call our patient’s providers if a patient has refused a medication to address medication regimen changes.

Pill boxes and Unit dosing:

• We will take the time to set up pill boxes for patients.
• Color coding, medication alarms, and specialty labeling in many languages is available.
• We verify the patient understands proper use and storage.
• We can fill pill boxes on a weekly or monthly basis.
• Unit dosing is available per request.

Follow Up:

• We follow up with patients struggling with barriers to optimal compliance.
• We will work with case workers and social worker to help accommodate special patient circumstances.
• We will take the time to help our patient strive toward compliance.
